A form of health care in which a group of doctors looks after a roster of patients and are paid based partially on the size of that roster.

Groups of doctors, between seven and 30, give up their private practices and form a co-operative. In some cases, they physically move in together; in others, their offices are linked by computer. They hire nurse practitioners and other specialists to provide extended service."
